WELCOME to Arrhythmia and Cardiology of
Chattanooga
Dr. Gershon Perry, M.D.,
F.A.C.C. would like to welcome you to Arrhythmia and Cardiology of Chattanooga.
Our goal is to provide an extension of our office (physicians, nurses, technicians,
office personnel, office procedures) and information that our patients can use
and better understand their heart, how it works, its functions and care.

A Typical Visit To Our Office
Step 1. The first thing
a patient should do is bring their medication with them to the physician's office
to prevent any confusion on what medications you are presently taking or not
taking. If possible bring any questions that you may have written down to ask
during the session with the physician.
Step 2. Upon arriving at the office, patient should sign in at the reception
desk. The reason for signing in, is to help our office personnel to check off
your name on our appointment record and to pull your medical file folder so
the physician and nurse can review your records and notes that have been done
in the previous appointments. You may be asked for your insurance information
or make any changes that may have occurred since your last visit. You may be
asked to fill out an update sheet to see if anything has changed since your
last visit.
Step 3. A nurse or technician will perform any or most of the following to help speed up the time you have to be with the physician:
Weigh In - indicates
any unusual weight gain/loss and explanation of the reason for change
Blood Pressure Check - nurse will take a reading for the records and
for physician notes
Temperature Check - a normal temperature is 98.6 degrees fahrenheit (any
plus or minus variation will be noted in the notes to the physician)
Pulse Check - nurse will take your pulse by your wrist (average is 70
- 72 beats per minute for adult males; 78 - 82 beats per minute for adult females)
Pertinent Questions - history of any medical problems or disease, family
diseases, etc.
Statistics - allergic to any medications, previous surgeries, injuries,
etc.
Family History - any history of family causes of death, disease, etc.
Electrocardiogram - if required the nurse will monitor your heart and
record the information
Technician Visit - if you are scheduled to see one of our technicians
for a check of your pacemaker or any other device that has been assigned to
your health care
Step 4. If scheduled, the physician will see you. The physician will come in and look over your medical folder and any notes the nurse or technician may have made in relation to your health care. The physician will usually listen to your lungs at various locations while you take deep breaths. The physician will monitor your heart with a stethoscope and examine other areas for his information gathering session. At some time during this session you should ask the questions that you may have brought with you for this office visit.
Step 5. After the session, the physician will specify certain things for you to follow. It could be a new prescription, cease taking a certain prescription, change a prescription, increase or decrease a prescription dosage. The physician may ask you to follow certain steps that has been laid out for you to help maintain heath care. You may be given certain lab work to be done at the local lab testing facility. This information will give the physician internal data to work with that the naked eye and office testing methods can't determine. One example may be that you are deficient in one of your minerals, or an internal body organ isn't functioning properly. The physician may ask you to see your primary physician so that the remainder of your body can be examined and give each an overall view of your health status.
Step 6. After your session with the physician is over you will be directed to the reception desk for further instructions. It may be to schedule your next visit, payment or insurance co payment info, get a prescription form, information on what the physician has ordered for you, schedule lab work, etc.
Step 7. Make any notes, mark your calendar for any appointments to help remind you of the next office visit and write down any instructions the physician has given you to follow.

Miscellaneous information:
To make or reschedule an
appointment:
Please call our office during working hours at 423-778-7156. If you have a primary
care physician (PCP) please ask for you records to be copied and sent to us
via regular mail.
Insurance Questions:
If your insurance requires a referral, kindly ask your Primary Care Physician
(PCP) to furnish you a valid referral form. Please bring all of your insurance
cards with you. Contact us at ACC and ask to speak to anyone at the front
desk.
To meet or talk to the
physician:
To meet or talk to the physician: Please call to schedule an appointment and
specify what the appointment is for.
To follow up on your
test results:
If your test result requires intervention such as new medications or change
in current medications, we will call you. Otherwise they will be discussed with
you on your next office visit. A summary of pertinent blood tests will be mailed
to you once we have all of the results.
To get a refill of
your medications:
Please ask your pharmacist to
fax us a request for renewal of your medications. If you do not have a regular
pharmacy, either call us with the pharmacy's fax number or ask the pharmacist
to call us during regular business hours. Please do not wait until you run out
of medications.
Note: We cannot extend a prescription(s) for patients who do
not come for their regular checkups.

as pleasant as possible, however sometimes the number of patients can be more
than usual due to illness, scheduling conflicts, surgery complications, hospital
emergencies and any other reasons that sometimes causes a conflict with our
normal routine. We apologize for this situation and please forgive us for any
inconveniences. We value you as though your a part of our family."
